Mini-grant funding available for diabetes education

The Florida Department of Health, Division of Community Health Promotion, is partnering with WellFlorida Council to manage mini-grant distribution and provide technical assistance and expert mentoring services to diabetes self-management education (DSME) programs in Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Putnam, Sumter, Suwannee and Union Counties. The purpose of this funding is to expand access to DSME programs for people with diabetes. Funding is available in six categories (see list below). An applicant organization may apply for one or more categories, but must demonstrate need for each category and ability to perform the responsibilities of each category applied for.

CATEGORIES

Category A: Getting Ready to Develop a DSME Program. Maximum award: $5,000.

Category B: Training/ Professional Development for DSME Staff. Maximum award: $10,000.

Category C: Building Infrastructure to Provide DSME Services. Maximum award: $10,000

Category D: Becoming an Accredited/ Recognized DSME Program. Maximum award: $5,000.

Category E: Establishing DSME Satellites. Maximum award: $2,000.

Category F: Sustaining DSME Programs. Maximum award: $2,000.
