Veterans share their health information through MyHealthStory

The Veteran Initiated Electronic Care Coordination (VIECC) project at the North Florida/South Georgia VA Health Care System aims to improve care coordination of Veterans receiving healthcare from civilian providers. This project is part of a larger multi-site national pilot project that trains Veterans to use the VA’s Personal Health Record portal, My HealtheVet, to create a Continuity of Care Document (CCD) that can be shared with their civilian providers. The CCD is a summary of their VA health record and includes information such as medications, immunizations, recent lab results, allergies, and history of procedures and encounters. Patients are asked to share a copy of their CCD with civilian providers through the Web-based platform MyHealthStory (powered by RelayHealth technologies).

MyHealthStory, through CommunityHealth IT, is a secure personal health record for patients and an integrated longitudinal medical record with health information exchange tools for healthcare providers, practices and institutions.
