Columbia and Hamilton County Community Health Survey

The Florida Department of Health in Columbia and Hamilton counties, in partnership with WellFlorida Council, the local health planning council for North Central Florida, are encouraging county residents, healthcare providers and others to share their views on health by taking a 10-minute survey. The surveys are available here. Participants will be entered to win a $25 gift card.

The surveys will be open from Nov. 1 to Dec. 17. The Community Member Survey is for residents and individuals who live or work in Columbia or Hamilton county. The Provider/Partner Survey is for healthcare providers, nonprofits, government entities, service organizations, social service agencies and schools serving the area. Both are available in English and Spanish, and hard copies of all the surveys can be obtained by emailing cabarca@www.wellflorida.org.

Survey results, along with research and health-related data, will be used to assess the region’s most pressing health needs. This information will be made available in April in the report Columbia and Hamilton County Community Health Needs Assessment 2019.
