Pilot for a Coordinated Intake and Referral System in Alachua County

Healthy Start of North Central Florida was recently awarded a 21 month grant from the Florida Association of Healthy Start Coalitions to plan for and pilot a Coordinated Intake and Referral System (CI&R) in Alachua County. More than 10 partners have joined together to create a plan that will provide pregnant women and parents of young children with access to a family partner who will work with them to assess their strengths and needs and refer them to the services or resources they may need. The planning process will end in October and the pilot will be implemented  from October 2016-September 2017 at the Mother/Baby Unit at UF Health and at a local pediatrician office.  Family partners will work with families at each of these sites to ensure that they are linked to the resources and services they need to help their babies and young children thrive.  For more information about this new grant please contact Mona Gil de Gibaja at mgildegibaja@welllforida.org.
